RETROSPECTIVE ANALYSIS OF THE MORBIDITY STRUCTURE AND DISEASES PREVALENCE OF THE GENITOURINARY SYSTEM AND THEIR GENDER CHARACTERISTICS

Abstract

Background. Analysis of medico-demographic indicators dynamics in Ukraine reveals an unfavorable demographic picture with signs of depopulation phenomena, which actualizes the solution of priority tasks for preserving the reproductive health not only of women of childbearing age, but also of men. The purpose of the study was a retrospective analysis of the formation of the structure and levels of morbidity, the prevalence of diseases of the genitourinary system among the population of Ukraine as a whole and the male population, in particular. Materials and methods. The materials for the study were medico-demographic data of the official statistical reporting on the incidence and prevalence of diseases by the class of diseases of the genitourinary system in the section of the main administrative territories of the regions of Ukraine and the city of Kyiv for the period 2010- 2017, with distribution by gender. Research methods used: systemic approach, bibliosemantic, medical-statistical, graphic analysis, content analysis. Results. The analysis of the gender aspects of the formation of morbidity by the class of diseases of the genitourinary system revealed a significant predominance of both the absolute number of cases of newly established diagnoses of this class of diseases in women (in 5.5 times (p<0.001)), and the relative indicators of morbidity by the class of diseases of the genitourinary system in calculated per 100,000 of the relevant population (4.5 times (p<0.001)). The structure of the morbidity of women and men according to the specified class of diseases is given. Regional features of the morbidity of the population of Ukraine according to the class of diseases of the genitourinary system were manifested by its highest levels according to the results of 2017 in Dnipropetrovsk (8,209.7) oblast and Kyiv (6,980.5 per 100,000 population). The analysis of the dynamics of the prevalence of diseases of the genitourinary system revealed an increase in the specific weight of this class in the general structure of the prevalence of diseases among the population of Ukraine from 3.18 % in 2010 to 3.5 % in 2017. Regional features of the prevalence of diseases of the genitourinary system were traditionally manifested by the highest values of the indicator in Kyiv (16,270.5) and Dnipropetrovsk region (15,870.7) with the lowest value of the indicator in Luhansk (4,906.6) and Zakarpattia (5,745.6 per 100,000 population) regions. In the structure of the prevalence of diseases of the genitourinary system in men, the following had the largest specific weight: diseases of the prostate gland (35.6% or 1,810.9 per 100,000 of the relevant population); kidney infections (22.7% or 1,155.3 per 100,000 of the relevant population); chronic pyelonephritis (21.1% or 1,071.0 per 100,000 of the relevant population). The highest prevalence levels of prostate diseases were traditionally found in Kyiv (932.0 and 5,156.8, respectively), Mykolaiv (883.8 and 2,833.9 respectively) and Kharkiv (749.0 and 3,107.3 per 100 thousand male population, respectively) regions. Conclusions. Monitoring of regional, gender-specific characteristics of the formation of indicators of morbidity and prevalence of diseases of the genitourinary system can become a well-argued information basis for the formation of regional programs and measures to preserve and strengthen reproductive health among men and women of Ukraine.
